To apply:
1. Download, print out and complete the enrollment form.
2. Be sure to sign and date enrollment form.
3. Download, print out and complete NASCOE membership card with office information.
4. Mail enrollment form and card to:

JM Marketing will fax your office with new payroll deduction upon receipt of your enrollment form and card.

Please read carefully!

SPECIAL NOTE: To terminate your existing dental deduction:

• You, the employee, must terminate your prior dental deduction.
• J M Marketing will fax your office with a form to send your state office to start your new dental plan deduction.
• It is up to you to notify your state payroll deduction to stop your prior deduction and start the new one. (J M Marketing cannot stop your prior deduction.)

If you intend to terminate your prior Dental Plan and purchase the NEW NASCOE sponsored Dental Plan through Allstate/American Heritage, do not wait or you may find yourself locked into another year of payments. It is our understanding you must do this by December 10th.
